Training Statements

Please read each statement carefully and indicate your agreement.

1. I accept that Diver Training has additional risks above and beyond those of many other recreational sports.

2. I fully understand the need for self discipline and additional care when learning or practicing diving skills, at the course level I am taking as stated on this form.

3. I acknowledge and agree that at this level of training I must be fit to dive and I must pay strict attention to Instruction and Safety Briefings. I will follow the directions of staff & crew.

4. I fully understand and agree that I am paying for the course and training, equipment hire from the facility/dive school or Instructor and my tuition. I may not be paying for the e-certification or ID Card (which is extra), unless my instructor informs me otherwise.

5. My ITDA-IHMP Instructor and staff have my personal safety as their first priority — NOT the certification process or training program. I agree that I am fit and healthy to perform the training required for this level of certification.

6. If I fail to meet the standards of training required for this certification level by the ITDA International Training Board, or should I endanger the staff or other divers by behaviour, distraction or attitude, my instructor may fail me, suspend or expel me from the course.

7. My ITDA Instructor is at liberty to make further charges as required for me to meet standards, or fail me without a refund or any recourse to instructional obligation or Certification.

8. I may also be asked to complete a Medical Form or Statement and/or a waiver (USA normally).

9. Thank you for choosing ITDA for training and certification. We aim to please and to keep you safe during training. Once certified, we highly recommend you gain as much experience as possible under supervision or leadership until you feel 100% confident, especially in self-rescue and being self-reliant. Verification Data Consent & Release

I, the undersigned certification holder, hereby grant ITDA-IHMP International express and informed permission to publish my photograph, full name, date of birth, and certification level(s) on the ITDA-IHMP public verification register (itda-member.org/verify).

Purpose: This information is published so that third parties — including dive centres, equipment hire facilities, guided tour operators, employers, and other verification parties — can confirm my certification status and identity, as required by industry safety standards and age-restricted activity regulations.

What is published: My photograph (passport-style), full legal name, date of birth, certification level, issue and expiry dates, certifying instructor name, and training facility.

Who can access it: Any member of the public who searches by my name or scans the QR code printed on my certification card. Full identity details (photo, date of birth) are displayed when a verification is accessed via my ITDA Number or QR code. For anonymous name searches, full identity details are shown only if I have granted this consent; otherwise, name and certification level are shown with a notice directing the verifier to use my ITDA Number or QR code.

Right to withdraw: I understand I may withdraw this consent at any time by contacting ITDA-IHMP HQ, and my personal identity details (photo, date of birth) will be withheld from anonymous name searches. Withdrawal does not affect the validity of my certification.

Data retention: My certification data is retained for the duration of my active certification and the standard archival period thereafter, in accordance with the ITDA-IHMP Privacy Policy and GDPR.
